I have no idea on how to install a custom map into train fever
how do you install a custom map

Just go to your "Train Fever" dirctory (C:\Program Files (x86)\Steam\steamapps\common\Train Fever) and create a new folder maps.
For each map a separate subfolder is needed. Inside this subfolder you'll have to copy heightmap.png and info.lua from the archive you downloaded.Normally there will be alreaday a subfolder inside the rar/zip archive. So you can use the extracted folder. But make sure you do not accidentally a subfolder inside the first subfolder. Train Fever won't find them

What Train Fever Version do you use? Did you update your Game to the latest version 7753?

When you create a new game you'll be able to select your custom map this way. But there is no new preview Picture. -

You'll have to switch to basic settings first

have you tried deleting the map and re-installing it? And which custom map are you using?

in the map downloads 3rd page over it's the pure flat map

I just tried installing the map, it showed up but when I started a new map my game crashed without sending a crash report, weird.
Have you tried installing a different map?

If you like flat, but more realistic map, you might have a look at "Hovedstaden" map of the nordic maps library: Nordische Kartensammlung - Nordic Maps Library
This map is from Danmark, has a nice amount of water but is quite flat (no mountains).

No. You can play it without any mods/dlcs.
With nodic dlc, the landscape will only look different because of more trees, other textures and shaders.

Theres also this mod, it makes maps generally flatter then they normally are, not completely flat but flat enough
